Please be aware that your test results from the Lateral Flow Test (LFT) are being recorded to help the Trust safeguard you , our patients and your family and colleagues. Results you provide via the PATH EKS App may be shared with your Line Manager, Occupational Health and other health bodies, as well as Public Health England and NHS England for reporting purposes

We rely on the following specific conditions in Article 6 of the GDPR to process your information: 6(1) (e) ‘…for the performance of a task carried out in the public interest or in the exercise of official authority…’ and 9(2) (h) ‘…medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems…’. We will also share your information as indicated with partner organisations as required in line with the COPI Order dated 20th March 2020 and its subsequent extension order of 29th July 2020: Covid-19 – Notice under Regulation 3(4) of the Health Service Control of Patient Information Regulations 2002.


In line with the Data Protection Act 2018 and GDPR we will retain the data entered on the app for up to 6 years following the end of your employment with the Trust. You have the right to access the records the Trust holds on you upon request. If you wish to request a copy of your completed form or any other data the Trust holds on you, please contact the Subject Access Request (SAR) Team on dhc.sar.enguiries@nhs.net
